Registration
Registering is quick and easy at the kids PAD! You can register online, come in and register, or download the forms and bring them in on your first visit. You will need to complete the Registration form, Parent Contract, Emergency Contact/Parental Consent Form, Parent Health Statement, and Child Health Assessment. The Child Health Report will need to be filled out by your child's physician and brought back to the center within 30 days. When you bring these forms into the kids PAD and pay the membership fee, we will take your child's picture, as well as the picture of all authorized pick-up persons. This is a security measure that also expedites the process of dropping off and picking up your child.
We encourage parents to bring their children in for a tour of the kids PAD and to meet our staff before their first visit. Some children may immediately feel comfortable and walk right in on their first visit, and other children may need more time to adjust to the separation from their parents. If you think your child may need more time adjusting, we suggest you spend 10-15 minutes in the center with your child before leaving. Please share any special instructions or concerns regarding your child with our staff. We will work to make sure your child feels comfortable and secure while at the kids PAD. We want every child to have a fun and positive experience playing at the kids PAD!
